CA125是1981年由Bast等從上皮性卵巢癌抗原檢測(cè)出可被單克隆抗體OC125結(jié)合的一種糖蛋白，來(lái)源于胚胎發(fā)育期體腔上皮，在正常卵巢組織中不存在，因此zui常見(jiàn)于上皮性卵巢腫瘤（漿液性腫瘤）患者的血清中，其診斷的敏感性較高,，但特異性較差。黏液性卵巢腫瘤中不存在,。80%的卵巢上皮性腫瘤患者血清CA125升高,，但近半數(shù)的早期病例并不升高，故不單獨(dú)用于卵巢上皮性癌的早期診斷,。90%患者血清CA125與病程進(jìn)展有關(guān),，故多用于病情檢測(cè)和療效評(píng)估。95%的健康成年婦女CA125的水平≤40U/ml,，若升高至正常值的2倍以上應(yīng)引起重視,。另外CA125也可見(jiàn)于結(jié)核性腹膜炎患者的血清檢查中，且CA125水平呈數(shù)十倍升高,，在卵巢癌術(shù)前應(yīng)明確排除結(jié)核性腹膜炎可能,。

診斷檢查
病原體檢查
取自患處的標(biāo)本作直接涂片或培養(yǎng),，涂片可見(jiàn)菌絲或曲霉菌孢子，培養(yǎng)見(jiàn)曲霉菌生長(zhǎng),。曲霉菌是實(shí)驗(yàn)室常見(jiàn)的污染菌，必須反復(fù)涂片或培養(yǎng),，多次陽(yáng)性且為同一菌種才有診斷價(jià)值,。
病理組織檢查
取受損組織或淋巴結(jié)活檢，可根據(jù)真菌形態(tài)確診,。尤其對(duì)播散性曲霉菌,，可及時(shí)作出診斷。
治療方案
一般治療
1,、積極治療原發(fā)并,，去除病因。
2,、嚴(yán)格掌握抗生素,、糖皮質(zhì)激素和免疫抑制劑的用藥指征，盡可能少用或不用這些藥物,。
3,、加強(qiáng)護(hù)理和支持療法，補(bǔ)充維生素和微量元素,。
抗真菌治療
1,、制霉菌素
（1）局部用藥：可制成油劑、霜?jiǎng)?、粉劑,、溶液等，濃度為含制霉菌?0萬(wàn)U/g或/ml基質(zhì)，依患者具體情況援用一種劑量一型局部涂擦,，每日2—4次,。
（2）口服：腸道念珠菌病可給予制霉菌素口服，新生兒每日20—40萬(wàn)U,，2歲以下每日40—80萬(wàn)U,，2歲以上每日100—200萬(wàn)U，分3—4次飯前服用,，療程7—10日,。口服不易吸收,，全部由糞便排出,。不良反應(yīng)有抗原抗體、嘔吐,、輕瀉,。
（3）霧化吸入：適用于呼吸系統(tǒng)念珠菌病，制霉菌素5萬(wàn)U溶于2ml0.9%氯化鈉溶液中霧化吸入,。
2,、二性霉素B為多烯類抗生素
與真菌胞膜上的固醇類結(jié)合，改變膜的通透性,，使菌體破壞,，起殺菌作用。是目前治療隱球菌病,、組織胞漿菌病和全身念珠菌病的*藥物,，對(duì)曲霉素菌病效果較差。
（1）靜脈滴注：開(kāi)始義用小量,，每日0.1mg/kg,，如無(wú)不良反應(yīng)，漸增至每日1—1.5mg/kg,，療程1—3個(gè)月,。靜注時(shí)用5%葡萄糖液稀釋，濃度不超過(guò)0.05—0.1mg/ml,，緩慢靜脈滴注,，每劑不少于6小時(shí)滴完。濃度過(guò)高易引起靜脈炎,，滴速過(guò)快可發(fā)生抽搐,，心律時(shí)常、血壓驟降,，甚至心跳停搏,。
（2）椎管內(nèi)注射或腦室內(nèi)注射：限于治療隱球菌性膜的病情嚴(yán)重或靜脈滴注失敗的病例,。兒童鞘內(nèi)注射，*0.1mg用蒸餾水（不用0.9%氯化鈉溶液）稀釋,，濃度不超過(guò)0.25mg/ml（偏稀為宜）或?qū)⑺幬锱c腰穿時(shí)引流出的腦脊液3—5md混合后一并緩慢注入,。以0.5mg為止不低超過(guò)0.7mg。療程一般約30次,，如有副作用可減量或暫停用藥,，腦脊液內(nèi)藥物過(guò)多可引起蛛網(wǎng)膜炎而腦脊液細(xì)胞增多，暫時(shí)性神經(jīng)根炎,、感覺(jué)消失,、尿潴留、甚至癱瘓,、抽搐,，如及早停吆五，大多能緩解,。
